The hourly rate for this position is $64.98 / hour (includes in lieu of benefits and vacation)
Primary Responsibilities
Reporting to the Executive Director Clinical Strategy and Projects, the Project Manager Community Health is responsible for managing all aspects of the joint initiative and advancing the objectives of the Memorandum of Understanding (MOU) between the Town of Georgina, Southlake and the NYSS OHT to bring care closer to the residents of Georgina and supporting strategic health initiatives for Southlake.
This includes project planning and management, research and data collection, support of procurement activities, community and provider engagement, monitoring and progress reporting.
This position will serve as a central point of contact for all external and internal partners and will ensure the successful implementation and advancement of the MOU objectives, and associated initiatives.
The role also involves building and maintaining community relationships to foster collaboration and support for health initiatives and aligning efforts to support Southlake’s strategic goals.
1.Memorandum of Understanding (MOU) and Associated Initiative Development :
- Develops detailed project plans, timelines, and budgets related to the MOU and associated initiatives.
- Leads the establishment of project governance structure.
- Facilitates the development and signing of necessary agreements and contracts, including funding and data sharing agreements.
- Organizes and leads project kickoff meetings and ensures all stakeholders are aligned on project goals and objectives.
- Manages the MOU work plan development and implementation process, including liaising with internal and external stakeholders, and managing project artifacts (scope, budget, timelines, risks, stakeholders, communication, etc.).
- Leads the research and data gathering related to the objectives outlined in the MOU and associated initiatives; including managing 3rd parties to perform environmental scans, conduct data analysis, prepare presentations and reports.
- Works with 3rd parties to coordinate and facilitate discussions, to conduct public information sessions, prepare and administer surveys and other required materials, collects and analyses data.
- Presents progress and findings to stakeholders as required, including a formal bi-annual presentation to the Town of Georgina Mayor and Council.
- Develops and maintains professional relationships with internal and external stakeholders (e.g., Northern York South Simcoe Ontario Health Team (NYSS OHT), Health Georgina, Georgina Nurse Practitioner-Led Clinic, York Region Health Services, other municipalities, broader public sector organizations, Town’s departmental leads, etc.).
- Monitors project progress to ensure accountability and track scope / budget / milestones / etc. towards goals and targets.
- Works to increase health-oriented service capacity by planning and overseeing the development of new healthcare facilities and optimizing existing spaces in Georgina to enhance accessibility and care provision for residents.
- Facilitates data-sharing initiatives that leverage each organization’s information while ensuring compliance with privacy regulations.
- Explores and establishes partnerships with other organizations to share and combine data, providing a comprehensive understanding of the community's health and social care needs.
2.MOU and Initiative Management and Leadership :
- Leads the implementation and advancing of the objectives of the MOU and associated initiatives.
- Collaborates and coordinates the participation of stakeholders such as NYSS OHT, Health Georgina, the Georgina Nurse Practitioner-Led Clinic, York Region Public Health, etc.
- Offers expertise to stakeholders relating to the MOU and associated initiatives.
- Leads the development of policies, plans and strategies, as well as procedures and guidelines related to the MOU.
- Leads and manages community engagement activities related to the implementation and advancing the objectives of the MOU and associated initiatives.
- Recommends project team members based on corporate impact, relevant experience, and resource availability / corporate capacity.
- Manages procurement activities as required.
- Manages contracts and vendors as required.
- Identifies and develops implementation plans for evidence-informed health programs and services tailored to meet both current and future health and social care needs of the Georgina community.
- Fosters health equity by identifying opportunities to elevate all voices in every aspect of work undertaken through this role.
- Collaborates with system partners to integrate and streamline care delivery closer to home.
- Leads the expanding and enhancing of existing programs and services provided by Southlake and the NYSS OHT in Georgina, while actively promoting and educating the community about these offerings.
- Actively pursues and advocates for funding and investment from various levels of government and other stakeholders to support health initiatives.
- Collaborates in any additional areas identified as mutually beneficial to meet the goals and objectives of the Memorandum of Understanding (MOU).
- Works with other entities (e.g., OHTs, hospitals, and local service providers) to improve collaboration, develop strategies, and strengthen healthcare service provision across Georgina.
3.Human Resource Management and Team Leadership Responsibilities :
- Manages diverse project teams including both internal and external stakeholders and third-party consultants in the implementation, management and advancing the objectives of the MOU, including selection, scheduling, assigning, and monitoring work, determining training and development needs, coaching and mentoring, performance against determined measures and outcomes.
- Ensures compliance with the Occupational Health and Safety Act and other applicable legislation.
- Provides visible and positive leadership to staff developing and nurturing a work environment that is inclusive, respectful, and motivating for staff.
- Establishes and supports health and wellness initiatives through effective collaboration with community and health and social care partners / providers, incorporating engagement feedback into advocacy efforts.
- Ensures representation of Georgina’s diverse population in all health and wellness activities, fostering open communication and building trust with the community.
- Serves as a liaison between the community and the organization to enhance relationships and promote mutual understanding and cooperation.
4. Other Duties :
- Performs other duties as assigned in accordance with the areas of the MOU and associated initiatives
- Acts as a member of various strategic, corporate, and external committees and teams, providing advice on relevant initiatives / policies as required.
- May occasionally be required to work outside of normal business hours.
- Proactively manages high priority issues requiring attention.

Qualifications
- University Degree or College Diploma related to Health Care, Business or other relevant discipline from an accredited University or College
- Course training in Health Care and Health Service, or other health related topics
- Project Management Professional certification and / or training in this area is considered an asset
- Training in Facilitation or Community Engagement considered an asset
- Five (5) years of demonstrated experience related to Strategy and Plan development and Project Management experience
- Direct experience related to health care initiatives and project management
- Direct experience in community engagement and outreach activities
- Experience in the development and execution of planning documents that align with strategic vision, and writing detailed proposals, business / operational plans, and reports
- Demonstrated experience in leading and collaborating with cross-functional and multi-disciplinary teams, both internal and external to the organization
- Extensive experience with complex corporate initiatives and cross-departmental projects
- Previous experience working in municipal government in a cross-corporate capacity and having had supervisory or leadership experience are considered assets
