Role Summary :

The Community Health Manager plays a critical role in enhancing healthcare outcomes through the oversight and continuous improvement of care management systems.

This role involves developing key performance indicators (KPIs) for Care Managers, standardizing best practices, and collaborating with interdisciplinary teams to advance evidence-based patient care.

As a leader, you’ll analyze operational and budgetary data, guide performance improvement strategies, and ensure alignment with organizational objectives.

Essential Responsibilities :

Analyze departmental data on operations, budgets, and staffing to identify trends and formulate action plans that improve service delivery and team efficiency.

Create and implement standardized practices for all Care Managers to drive consistency and quality in patient care.

Develop and monitor KPIs that support strategic initiatives, aligning them with broader goals and scorecard metrics.

Partner closely with organizational leaders in a collaborative operating model to foster success within the Care Management team.

Support operational needs across areas of responsibility, providing guidance to staff as necessary.

Lead interdisciplinary initiatives to establish evidence-based care standards and assess patient outcomes, encouraging staff engagement.

Design, execute, and evaluate performance improvement systems, using data insights to inform case management strategies.

Represent Care Management on various organizational committees, promoting team involvement and ensuring alignment with organizational goals.

Required Qualifications :

Bachelor of Science in Nursing (BSN) with a current RN license in Ohio, or a Master’s in Social Work / Counseling with an active Ohio license.

At least three years of experience in a hospital or inpatient care setting.

Three years of management or demonstrated leadership experience.

Comprehensive knowledge of clinically integrated networks, payment systems, managed care principles, infection control, patient care standards, disease processes, and the full continuum of care.

Desired Qualifications :

Proven experience in healthcare performance improvement and process development.

Skilled in interdisciplinary team leadership and KPI management.

Proficiency in data-driven decision-making to optimize patient outcomes.
