Schedule

Patient care
In this role, you will, in collaboration with the interdisciplinary health care team, be responsible for comprehensive assessment, care planning and education for patients and families with Diabetes. Your strong counselling skills coupled with well-developed analytical, problem-solving and interpersonal skills will assist you in the provision of quality patient care. 48_Quality Assurance Created with Sketch.
Quality assurance
You will practice in an environment where the emphasis is on professional practice and customer education, empowerment and encouragement. You will coordinate your services with the other staff in the Clinic, which may require day and evenings as well as potential travel to our satellite sites. In addition, you will participate in ongoing quality improvement and promotional activities, committees and the development of standards and policies as it relates to our Diabetes Program.

Qualifications
Current certificate of registration with the College of Nurses of Ontario; BScN Preferred. A minimum of two years recent experience in a diabetes education / diabetes management setting; Certified Diabetes Educator designation required. Knowledge of the South Asian & South East Asian culture and ability to speak in a South Asian and South East Asian language is an asset. Knowledge
Excellent knowledge of diabetes education / management best practices. Excellent knowledge of insulin and Oral Anti-Hyperglycemia agent (OHA) treatment modalities and associated demonstrated proficiency in insulin / OHA adjustment. 48_Creative Created with Sketch.

Demonstrated good attendance and performance records with the ability to maintain these same standards. Skills
Excellent interpersonal, communication, teaching and organizational skills. Certified Pump Trainer designation is an asset; Excellent knowledge of continuous glucose monitor and insulin pump management principles is preferred. Computer skills are essential. Ability to work independently and as part of an interprofessional health care team. Must have own transportation and the flexibility to work some evenings in various community settings.
