The NP will also provide longitudinal care, with particular focus on care coordination with different healthcare providers and specialists, co-management of common mental health conditions and concerns that patients may experience during this adjustment period, and helping patients and families navigate the adult health care system.
Lastly, the NP will also take part in quality improvement initiatives and / or research to further our understanding and approach in providing patient and family care during this important juncture in their healthcare journey, and for future advocacy initiatives.
Patients enrolled in The Ledcor Centre for Adolescent and Transitional Urology will reside from across British Columbia.
Thus, it is anticipated that some of the care delivery and coordination will be via virtual platform. The NP will have an on-site office at GF Strong Rehabilitation Centre with access to examination rooms and clerical support.
