Position Summary
Social Worker, Paediatric Complex Care Clinic
Your goal in this position is to provide appropriate interventions to patients and families in the Paediatric Complex Care Clinic through individual, family and group treatment modalities.
You will coordinate the utilization of resources to meet patients and families ongoing psychosocial and continuing care needs.
You will provide supportive counselling to parents to assist them in coping with the care needs of their child, the diagnosis of chronic conditions, and the impact of the complex care needs on their family.
You demonstrate flexibility and the ability to work under pressure. You have effective communication and interpersonal skills, excellent customer service skills, combined with the ability to function collaboratively as an interdisciplinary team player.
This position supports the hospital’s academic mandate by advancing care through teaching and learning and / or transforming care through research and innovation.
This position contributes to the shared responsibility of ensuring health and safety policies are followed to provide for a healthy environment for patients, staff, physicians, volunteers and families.
On a practical level, you will
- Participate in and or facilitate educational opportunities for social work students and the paediatric health care team.
- Continue with professional development to ensure practice is current and evidence based and meets best practice guidelines in caring for this population of patients and families.
- Demonstrate a commitment to continuing education and professional development to ensure practice is current and evidence based and meet best practice guidelines in caring for this population of patients and families.
- Maintain excellent knowledge of, and close liaison, with hospital & community resources, i.e., Community Care Access Centre, Social Assistance, Ontario Disability program, Drug Programs, etc.
- Advocate for the patient / family and community to ensure the patients’ rights are recognized and appropriate services are created / provided.
Qualifications
- Master of Social Work degree required
- Current registration with the Ontario College of Social Workers & Social Service Worker.
- Recent Paediatrics hospital experience
- Recent experience working with children’s aid society
- Recent experience with clients with post partum depression
- 1 year of hospital experience in a similar setting preferred
- Research experience / education an asset
- Student education / supervision an asset
- Computer proficiency required (i.e., MS Word & MS PowerPoint)

This is a Part Time Temporary position of 12 months with 8 hour shifts, days shifts in the Paediatrics Complex Care Clinic.
