Social Work in Long-Term Care

We have an immediate opportunity for a Social Worker at Nithview Community in New Hamburg and Greenwood Court in Stratford.
The primary goal of the Social Worker is to utilize a person-centred approach to care and to work collaboratively with residents, caregivers, and other team members at both our long-term care homes Greenwood Court and Nithview Community.
This role is also accountable for overseeing Spiritual Care needs for residents, planning liturgies and worship services for observances of religious feasts and supports multi-faith programming and education.
Additional functions include coordination of pastoral care at the end of life and support to families and staff dealing with grief and loss.
The ideal candidate will be self-directed, energetic, and creative, able to respond to the diverse spiritual needs of patients, families, and staff.
They will serve as an integral member of an interdisciplinary care team ministering to culturally diverse populations in a faith-based, publicly funded healthcare context.
The Social Worker will hold individual :
Qualifications :
- Relevant Degree / Diploma (SSW, BSW, MSW)
- Current registration with good standing with the Canadian Association of Social Workers and / or Ontario College of Social workers is mandatory
- Previous experience in a long-term care home is preferred, knowledge of Ministry of Health and Long-Term Care legislation and standards preferred
- Minimum one-year experience preferably in a health care setting
- Experience or training in Palliative Care is and asset
- Experience in Medical Social Work is an asset
- Possess strong interpersonal, communication, organization, and administrative skills.
- Experience in program planning / implementation meeting the needs of all residents within the continuum senior care environment
- Have experience and / or willingness in working with seniors and understand LTC Homes Act / Regulations, MAID, and other applicable legislation
- Computer literacy is a requirement. Familiarity with the Point Click Care software is an asset.
- Knowledge of Ministry of Health and Long-Term Care legislation and standards preferred, experience working with dementia and palliative residents
- Great communicator well organized team player with clinical skills in care planning, supportive therapy, crisis intervention and grief counselling, preferred
- Superior client relationship skills and a passion for advocating for residents' rights and person-centered care
- Strong interpersonal and organizational skills
- Excellent interpersonal and communication skills (verbal and written), to work effectively with team members, residents, family members and other stakeholders
