JobSummary : Were seeking compionate and dedicated Personal SupportWorkers (PSW) Health Care Aides (HCA) or Continuing Care istants(CCA) to join our awardwinning team.

In this role youll providedirect personal support and care to patients addressing theirphysical intellectual emotional social and spiritual needs inaccordance with our patientcentered policies andprocedures.

KeyResponsibilities :

Providepersonalized care to patients including istance with ambulationfeeding bathing and grooming

Implement and follow care plans as directed by nursing staff

Doentpatient care activities and observations accurately and promptly

Communicate effectively with patients families and other healthcareteam members

Createa safe comfortable and dignified environment for patientsParticipate in ongoing training and professional developmentopportunities

Qualificationsand Ss :

Completion of a PSW Program or HCA / CCA certification that meetsappropriate standards

Experience in Long Term Care or community home care setting(preferred)

Knowledge of dementia and palliative care (preferred)

Familiarity with care planning systems and doentation

Strongcommunication ss and ability to interact positively with staffresidents / patients and families

Proficiency in reading writing and understanding verbal and writteninstructions in English

Abilityto work independently especially in a Community Home Care setting

Accessto a reliable vehicle for community home care(required)
