Job Description
North York Toronto Health Partners (NYTHP) is a collaborative partnership with patients, primary care, and community partners (such as Circle of Care), a first-generation Ontario Health Team (OHT), driving the development of a locally integrated health system to serve the community.
The Coordinator, Community Engagement will play a critical role in designing and delivering community engagement activities for NYTHP to inform health system transformation.
They will serve as part of the NYTHP backbone team, hosted at Circle of Care and operate in a matrix style environment responsible to both NYTHP and Circle of Care.
This role will also involve coordinating and managing the event logistics and overall project management of Community Health and Information Fairs (CHIFs), which aim to increase community access to healthcare services and supports for equity-deserving populations.
In this role, you will support health system change at NYTHP, in accordance with our NYTHP Patient, Family, and Caregiver Partnership and Engagement Strategy and NYTHP Equity Framework.
You will be working to support NYTHP members from across the partnership, as well as actively engaging in the design and delivery of various community engagement activities and initiatives.
Through this role with Circle of Care and NYTHP, you will be working to re-design healthcare and foster innovation and creativity, in response to the needs of the initiatives you support, and the community we as an organization and a partnership are supporting.
We are looking for : a full-time, contract Community Engagement Coordinator (1 year) Salary : Commensurate with experience Hours of Work : 34 hours / week.

Community Engagement
- Develop, coordinate, and implement community engagement strategies and activities across NYTHP and North York community
- Organize and facilitate community or public consultation sessions, workshops, town halls, and focus groups, from beginning to end, to gather input and feedback
- Lead the planning, coordination, and execution of community engagement activities, including scheduling, booking locations, developing and distributing communication materials, facilitating the session, organizing honoraria, arranging additional services (.
translation, catering, transportation, , and communicating with stakeholders and community members
- Develop a process for capturing and storing community engagement feedback and insights
- Take ownership of all event reporting including event registration, attendance, success measures, and community engagement feedback and insights
- Identify online media opportunities that elevate NYTHP’s profile and identity in the community landscape
- Promote, develop, sustain, and expand relationships (including day-to-day relationships) with agencies, organizations, and community grassroots groups that provide services to diverse communities in North York Support evaluation activities related to community engagement including identification, implementation, and monitoring of improvement efforts as needed, aligned to existing best practices and evidence-based approached
Community Health and Information Fairs (CHIFs)
- Organize and deliver CHIFs from beginning to end
- Provide secretariat support to the CHIF Planning Team, including setting agendas, writing meeting minutes, and logging and tracking action items and key decisions
- Manage the overall project plan for CHIFs, ensuring milestones and deliverables are met
- Monitor and evaluate the progress of the CHIFs, identifying, addressing, and / or escalating any risks or issues that may arise
- Lead the planning, coordination, and execution of CHIFs, including scheduling, booking locations, communicating with vendors and exhibitors, managing and ordering inventory, transporting equipment or supplies to the event location, and managing attendee flow
- Provide day-of planning, logistics, coordination, and event-related troubleshooting at CHIFs
Additional Roles and Responsibilities
- Support on-going NYTHP initiatives, as needed
- Assist select NYTHP Committees and teams, and sit on relevant NYTHP Committees and teams
- Prepare regular reports for internal and external stakeholders
- Support engagement meetings / events and cross organizational meetings where appropriate through event planning and coordination efforts
- Assist with preparing a wide range of materials including agendas, minutes, invitations, reports, presentations, analyses, and other related documents for working groups and project touch-bases
- Support engagement with community and health system partners and community partners related to initiatives

Qualifications
- Undergraduate degree from a recognized University program or equivalent,
- Experience coordinating and supporting projects
- Experience with community engagement, ideally within the health care sector
- Experience working with high-need, complex, and diverse priority groups in the community health setting
- Ability to adapt to change; high level of flexibility and reliability
- Strong organizational and time-management skills
- Excellent verbal and written communication skills, with high attention to detail
- Strong relationship management and community engagement skills
- Ability to work as part of a team and independently
- Comfort with ambiguity
- Proficiency with Microsoft Office, including Word, Excel, PowerPoint, Outlook, and Forms
