Job Description

The Clinical Resource Nurse (CRN) has demonstrated clinical expertise in both rehabilitation and geriatric settings. The candidate is required to have past experience as a Clinical Resource Nurse, demonstrated leadership abilities, excellent communication skills, excellent attendance and commitment to quality improvement activities. It is a requirement that the CRN supports the Patient Care Manger and Professional Practice Clinician with monitoring of quality metrics through CRN status board, ensuring timely and smooth patient flow by supporting the Estimated Date of Discharge (EDD) initiative, and leading huddles as required.

On a day to day basis, the Clinical Resource Nurse is responsible for supervising and assisting nursing staff, as well as providing administrative support and patient care. Regular duties include reviewing rehab referrals, directing the admission, discharge and general flow of patients, and assigning nurses to patients taking into consideration RN and RPN skill mix. The CRN also coordinates and facilitates interdisciplinary team rounds. While daily goals must be met, the successful candidate is able to prioritize and adapt during emergencies. The Clinical Resource Nurse also works with the care team using a patient and family centered approach to create a plan of care that is individualized to a patient's needs.

  • Current Registration with the College of Nurses of Ontario required
  • BScN preferred
  • Previous clinical resource nurse experience preferred
  • Minimum of 3-5 years of clinical experience required
  • Clinical expertise in a Med / Surg setting required
  • Participates on a regular basis in in-service and educational programs / upgrading
  • Demonstrated ability to mentor, support, advise and lead others
  • Models a high level of professional practice related to clinical and interpersonal skills
  • Demonstrates conflict management and conflict prevention and resolution skills
  • Superb problem-solving skills
  • Excellent communication skills both written and verbal
  • Strong organizational skills
  • Acts as a professional role model for his / her nursing colleagues
  • Actively promotes a healthy workplace for all staff
  • Proven success in developing relationships with the interprofessional healthcare team
  • Must be able to meet the physical demands of the position
  • Satisfactory employment and attendance record required
