Position Overview :
The Nurse Practitioner will provide direct and indirect care to patients. This role involves providing comprehensive psychiatric assessments, diagnosis, and treatment to patients.
You will collaborate with interdisciplinary teams to develop and implement individualized care plans and ensure optimal mental health outcomes.
Setting : Out Patient Clinic
Population : Geratic population
Responsibilities :
Psychiatric Care : Provide psychiatric consultations, evaluations, counseling, and treatment in collaboration with physicians and the IDT team.
Assessment and Planning : Conduct in-person or telehealth psychiatric assessments and work with the IDT team to create individualized care plans.
Discharge Planning : Collaborate with physicians and IDT in planning discharge from acute or long-term care settings.
Referral and Oversight : Refer participants to psychiatrists as needed and oversee care in nursing homes, ensuring regular visits according to standards and participant needs.
Consultation : Serve as a consultant to the IDT, communicating participant changes and coordinating 24-hour care delivery.
Other duties as assigned
Collaboration : Work with other healthcare professionals to coordinate patient care and address complex needs.
Patient Advocacy : Support patients and their families in understanding and managing mental health conditions.
Documentation : Maintain accurate and up-to-date patient records in compliance with regulations.

Qualifications :
Master’s degree in Nursing with a Psychiatric / Mental Health emphasis from an accredited institution.
Certification as a Psychiatric Mental Health Nurse Practitioner.
Active and unrestricted state license as a Psychiatric Mental Health Nurse Practitioner.
State CDS Certification and DEA License.
NPI license
