- As an integral member of the Hospital leadership and patient care teams, the primary role of the Senior Clinical Program Director (SCPD) is to work in partnership with the program medical directors and includes : providing exemplary leadership and strategic guidance to the Senior’s Health and Ambulatory Care Program;
- ensuring excellence in the provision of patient care in day to day operations as well as longer term program planning for improvements;

On August 1, 2017, Providence Healthcare, St. Joseph’s Health Centre and St. Michael’s Hospital integrated to form a new health network known as Unity Health Toronto.

The SCPD of the Senior’s Health and Ambulatory Care Program leads the Providence Healthcare site specific Senior’s Health and Ambulatory Care Program with the opportunity to create and support future services at the St.
Michael’s Hospital and St. Joseph’s Health Centre sites. The SCPD of the Senior’s Health and Ambulatory Care Program will also have cross site programming with accountabilities spanning Senior Friendly Hospital services across all three sites.
The Providence site provides inpatient Geriatric Rehabilitation and Complex Care, and outpatient services including outpatient clinics and community outreach services, ambulatory care services, and the Adult Day Program and Caregiver Support Program.
All three sites provide senior’s care in both the inpatient and outpatient programs. We provide services to many living in poverty and facing many hurdles in achieving recovery and community re-integration and we are committed to supporting recovery through timely access to high quality, evidence informed comprehensive health and social supports.
Robust community based and community partnered programs and services are key the care we deliver.
The SCPD leads the strategic planning of all services in the Senior’s Health and Ambulatory Care Program and maintains responsibility for ensuring the efficient / effective delivery of the clinical program including all aspects of planning, human resource and financial management.
This involves facilitating program development and strengthening program vision, and promoting a culture of discovery and innovation in all clinical and non-clinical activities within the Programs and the development of external relationships with the community partners and stakeholders in patient care delivery.
Primary responsibilities of the SCPD role include :
- Providing senior operational leadership for the programs and services of the portfolio
- Directing and aligning clinical operations to effectively achieve corporate and program objectives.
- Providing leadership and oversight for the delivery of clinical care within the portfolio and ensuring that the quality of care properly meets the standards of care
- Planning and strategizing for changing circumstances and new program goals and objectives.
- Performing financial management and integrating financial considerations into programs in order to assist decision making.
- Establishing priorities for program resource allocations, including operating, capital, and renovation plans
- Championing a customer service approach to program goals and associated community relations.
- Enabling continued professional practice development of staff.
- Ensuring the continuous development of direct reports and management teams through leadership, education, coaching and performance management
- Supporting the advancement of Unity Health Toronto’s values based culture
- Collaborating with executive and peer leaders in fostering engagement and wellness of our people
- Participating and / or leading internal senior leadership committees
- Communicating with the executive team, internal partners, external stakeholder, government agencies, the MOHLTC as appropriate to ensure strategic advancement of the portfolio in alignment with Unity Health Toronto s strategic plans and the broader Ontario Health strategy
Overview of Role Requirements :
- Master’s Degree in nursing or health discipline required.
- Regulated Health Professional with certification in good standing from respective regulatory body.
- Minimum of 8 years in progressive management roles in a health care setting with at least five of those years in a Rehabilitation / CCC setting with Palliative care experience an asset.
- Experience in Leadership at a Director level an asset.
- Demonstrated knowledge and understanding of program management concepts to facilitate clinical and support services interactions within the programs.
- Demonstrated ability to develop and implement processes / systems to ensure effective and efficient program management infrastructure.
- Demonstrated leadership in continuous improvement, and in achieving operational excellence and fiscal management across the portfolio.
Lean experience and certification is an asset.
- Demonstrated values-based leadership competencies.
- Demonstrated experience in promoting a culture of learning, innovation, change and accountability within.
- Demonstrated skill level in collaboration, negotiation, conflict resolution, human and financial management.
- Demonstrated interpersonal skills and ability to establish positive working relationships with physicians, community partners, academic partners, and other programs and service providers.
