Job Summary
Performs as a member of the inter-professional Medicine team providing assessment, treatment, consultation and education services.
Skills & Abilities
- Two years’ experience as social work / discharge planning within an acute care setting.
- Demonstrated collaborative working relationship with an inter-professional health care team
- Supports ongoing learning through professional development and the coaching and mentoring of students
- Links patients, families, and / or significant others to support in the community.
- Assumes advocacy role in program on behalf of patient / caregivers.
- Provides short-term crisis and grief counselling for patient, families / caregivers and team(s).
- Counsels patient and family regarding hospital admission and discharge to provide continuity of care.
- Maintains documentation of clinical assessments, treatment plans and patient clinical progress.
- Maintains confidentiality of patient / family related to communication and documentation.
- Acts as a resource and liaison to community stake holders and facilitate referrals to community resources.
- Advanced knowledge and expertise in geriatric assessment, cognitive assessments.
- Excellent written, verbal and telephone communication skills to ensure effective communication with clients / families, colleagues, health care providers, community agencies, etc.
- Demonstrated ability to work both independently and as a strong team player.
- Demonstrates cultural sensitivity and diversity.
- Knowledge of governmental financial programs and resources.

Qualifications
- Masters of Social Work from an accredited university
- Membership in the Ontario College of Social Workers and Social Service Workers required.
