Job Description
Bayshore is looking for a passionate and caring Charge Nurses to join our amazing team at our Transitional Care Unit located in Toronto.
The address is 170 Emmett Avenue, Toronto, M6M 2J5. This is a full-time position on a 6 month contract with 12 hours shifts, rotation of day and night shifts and alternate weekends.
Working as a member of the inter-professional team, the Charge Nurse under the direction of the Clinical Manager and / or Area Director will provide specialized services in the management of patient care and ensure compliance with internal and external standards of practice.
This position requires a combination of days and overnight awake shifts, and the incumbent will have a regular work schedule.
Full-Time and Casual positions available.
DUTIES AND RESPONSIBILITIES
- Supports the needs of patient populations including innovative approaches to care delivery.
- Provides comprehensive care to the patients as part of an interdisciplinary / inter professional team focused on creation of a person-centered plan of care.
- Conduct patient assessments using the RAI-MDS tool, uploading data to hospital portal as required.
- Complete inventory and order supplies.Delegate the unpacking of shipments and restocking of kanban bins.
- Run and review reports from the Learning Management System, auditing of clinical compliance reviews and documentation related to patient care.
In addition, ensure that hourly rounds are completed by appropriate staff.
Ensure that care delivery is in accordance with Physician / NP orders. Direct the care delivery on the floor for unregulated caregivers.
Monitor, correct, and report any performance issues to the clinical manager as appropriate. Assess, mitigate, and report any client safety concerns (RL6).
- Supervise and support unlicensed employees to ensure compliance with all federal and provincial legislated standards of practice as well as company policies and procedures.
- Working with the Clinical Manager to assist with the integration of client satisfaction initiatives. Meet with all new admissions and conduct a meet and greet and provide a welcome folder to promote a positive client experience.
Ensure that all referred client concerns and complaints are satisfactorily addressed and documented in a timely manner.
Working in collaboration with the Patient Flow Coordinator, assist with incoming patient admissions by ensuring pharmacy services, medical equipment and supplies are available and any last-minute discharges are communicated.
Send morning report to Patient Flow Coordinator(s).
- Participate and encourage others with ongoing internal and / or external continuing education activities.
- Adhere to Bayshore Policies and Procedures.
- Participate in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System.
Participate in daily huddles and weekly quality huddles.
Participates in proactive Health & Safety activities while performing all duties. Is responsible for notifying immediately the Clinical Manager of any Health & Safety risks or concerns.
Responsible for immediately completing Accident Reports via RL6, for all direct reports who injure themselves on the job, monitor and support employees participating in modified work programs.
- Maintains confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel.
- Reviews and updates primary care activities in line with organization goals and projected outcomes.
- Promotes evidence-based practice using latest research-based guidelines.
- Ensures the provision of safe, evidence based, cost effective, patient and family centered care.
- Identifies and implement appropriate clinical documentation and record keeping practices.
- Keeps up to date with health-related policy and implement if deemed appropriate to impact patient care in a positive manner.
- Participates in ongoing quality monitoring and improvement.
- Participates in creating, delivering, and maintaining an exceptional quality program.
- Creates an environment that builds on the principles of integrated team approach.
- Other tasks as assigned and / or requested.
Job Qualification
Graduate from an accredited Registered Nursing Program; current registration with the provincial regulating body.
- 3-5 years of nursing recent experience with progressive responsibilities with some supervisory experience.
- Previous experience or knowledge of RAI tools
- Previous experience working with geriatric clients; diagnosing and staging of dementia.
- Effective verbal and written communication skills.
- Strong leadership skills, including identifying growth opportunities in others.
Strongly recommended :
- Experience with Clinical skills such as IV therapy, central lines, and wound care.
- Experience with Conflict Resolution / Customer Service Management & Dealing with Difficult Situations / Employees.
- Demonstrated ability to interpret company policies and procedures.
- Demonstrated ability to exercise independent judgment and to assess situations and problems efficiently and effectively.
- Experience with written documentation, care plans.
- Strong interpersonal, organizational, collaborative, and problem-solving skills.
- Ability to work well within an inter-professional team and to foster effective working relationships.

STANDARDS OF PERFORMANCE
The Charge Nurse must demonstrate ongoing competency in completing all duties and responsibilities of this job description, in response to changes by the regulating body, as well as agreed upon specific goals and objectives.
PERFORMANCE REVIEW
Performance will be reviewed on an ongoing basis with a formal review at the end of the six-month probationary period and subsequently, on at least an annual basis.
Input will be sought from employee, Supervisor, Branch Office, and Field employees, and in addition, will be based on ongoing client feedback and the extent to which performance meets expectations.
