Schedule
The successful candidate will be available to work between the days of Monday - Sunday in 8 hour day shifts, primarily between 08 : 30 - 16 : 30.
It is the responsibility of casual staff to provide reasonable availability to their department, inclusive of a minimum of two weekdays and two weekends within each 6-week period.
The successful candidate will also rotate to cover the Uxbridge site remotely. Please note, schedule is subject to change based on operational needs.

Patient care
Working as an integral member of the inter-professional team, you will play a critical role in providing social work services to patients and their families within the organization, in the acute care setting, with the exception of in-patient mental health.
There is a strong emphasis on psychosocial support, interdisciplinary problem solving, and liaising with appropriate members of the team to ensure patients and families are linked with necessary resources and supports.

The successful candidate will demonstrate exceptional leadership skills, a high level of professional integrity and accountability and play a key role in system development, as well as possess a strong level of understanding of care across the continuum.
Experience in short-term supportive counseling with patients and families dealing with difficult decisions and end of life concerns would be a valuable asset.

Recent related previous social work experience in a hospital environment. Experience in working with electronic documentation, preferably, Meditech.
