Wood's Homes is looking for one full-time Foster Care Support Worker to join our Foster Care Network.
Wood’s Homes Foster Care Network Program offers youth a stable home-like environment and the opportunity to transition from intensive residential treatment to more full-realized community living.
This program offers a number of foster home and group living arrangements, with the goal of ensuring that our youth are positively transitioning into adults and leaning all the skills necessary to living on their own as adults.
Role Summary
Reporting to the Manager of our Foster Care Network Program, the Foster Care Support Worker is responsible for therapeutic and support services to children, youth and their foster families in order to promote family-based care.
The provision of these services may include recruiting, training, matching and supporting foster families in addition to facilitating access to clinically based care, individual intervention, system co-ordination, and crisis support.
Although the primary focus is with Foster Care, the Support Worker may be deployed in education, residential or community services.
The Foster Care Support Worker will demonstrate due diligence in being exceptional in the delivery of high-quality services and in meeting the requirements of the Health and Safety Management System at Wood’s Homes.
Responsibilities
- Provides in-home and crisis support to foster families and youth.
- Reviews referrals, makes placement recommendations and attends meetings as required by immediate supervisor.
- Coordinates and facilitates various systems meetings including case conferences and planning meetings.
- Represents the program and networks with other programs and organizations to enhance understanding of the Foster Care Network.
- Completes all required health and safety training
- Reports workplace hazards, unsafe or unhealthy conditions, or incidents (including near misses, injuries, or equipment damage) to their direct supervisor.
- Shares the values of Wood’s Homes.
- Other related duties as required.
Qualifications
- Diploma or Degree required in one of the Human Sciences, Childcare, or a related field.
- Minimum four years’ experience working with children and / or adolescents and families.
- In-home support experience preferred.
- Must have a class 5 driver’s license and own transportation.
- Personal suitability.
