48 Application Deadline : November 22, 2024 by 11 : 59 pm Program Overview Intensive Services, In-Home Turning Point Youth Services (TPYS) is an accredited, non-profit agency serving youth (12 to 24 years of age) and their families in the east end of Toronto.

Turning Point’s mission is to empower youth to make positive change, an to accomplish this, it offers a continuum of services for youth and their families with a focus on mental health, housing / sheltering, and youth justice.

Turning Point Youth Services and WoodGreen Community Services announced on July 2, 2024 the integration of the two organizations.

The combination of these two agencies will enable a more effective and integrated approach to the provision of comprehensive support services for youth in the GTA.

The In-Home Intensive Program (IHIP) is for youth ages 12 to 17 and their families where there are more significant mental health, relationship, or behavioral concerns than can be addressed in counselling alone.

Service is provided more frequently by a counselor and / or youth worker in the home with the youth and parent / family.

The program aims to strengthen individual mental health, family cohesion, and the family’s ability to develop the relationship skills needed.

Youth and their parent / family are required to commit to an individualized plan of 6-9 hours / week. Referrals come through STRIDES Help Ahead centralized point of access.

What You Will Do Provide intensive services to youth and families in the community, including home-based services. Collaborate within the multi-disciplinary team to develop and implement the case management, interdisciplinary intervention, and treatment plans for youth and families Participate in the development and empowerment of clients through skill development of daily family and community life Provide individual, family and group support as indicated Provide case management services and outreach where needed to assist youth and families in the community Make regular home visits according to the youth’s individualized treatment plan Maintain and manage assigned client caseload to ensure achievement of quality / performance targets set by Manager Ensure that administrative duties are completed within the required timeframe, including the preparation and submission of reports and any other documentation required Monday to Friday schedule, with some early mornings and evenings.

Must be flexible for potential occasional weekend work based on client need. Perform other duties as required. What You Bring to the Team University or College degree and / or diploma in Child & Youth Work and / or Bachelor of Social Work.

Minimum 5 years of relevant experience working with adolescents, preferably in a treatment environment Demonstrated knowledge and experience providing evidence informed interventions to youth with acute (complex) mental health and substance abuse issues Demonstrated ability to effectively connect and work with youth, families and the community Demonstrated knowledge and experience with at-risk adolescents and young adults Experience in treatment appropriate goal setting;

experienced in teaching emotion regulation Demonstrated understanding of Family Systems, DBT, CBT, MI, attachment-based and trauma informed approaches to understand, contribute and support the implementation of the clinical model and treatment plans Demonstrated knowledge of appropriate intervention techniques that can be incorporated into the acts of daily living.

Experience in organizing and coordinating activities planned to ensure learning and the acquisition of appropriate attitudes and behaviours Demonstrated ability to exercise professionalism and sound judgment in conflictual, stressful and crisis situations Demonstrated ability for problem solving and critical thinking Strong case management skills Good computer skills and the capacity to write quality reports within required timeframes Valid Understanding and Managing Aggressive Behaviour certification.

Valid First Aid / CPR certification Completion of required suicide prevention and intervention training. What Will Set You Apart Previous family support experience an asset Ability to demonstrate an anti-oppression approach to practice and work from a client-centered, anti-racism, harm reduction, and trauma informed approach is required.

Demonstrate knowledge of issues affecting marginalized communities, policies, legislation, programs, and other issues related to scope of practice and social determinants of health.

Experience working with diverse and marginalized communities, including ethno-cultural and religious groups, LGBTQSS+ youth and youth adults, those impacted by poverty and those involved in youth justice.
