Overview

CarePartners is embarking on a new program in partnership with the Home and Community Support Services (HCCSS) Central East and we are looking for community-based support workers (CSWs) to work within the High Intensity Supports at Home (HISH) program.

These skilled CSWs will work withpatients and their families to support care while patients transition through the healthcare system.

The HISH program focuses in Wrap-Around Care so thatallteam members (including Nursing Therapy, Personal Support, Social Work and Community Support Services, etc.

are working cohesively to meet the needs of the patient.These CSWs will receive additional training and certification in Gentle Persuasive Approach (GPA).

In this exciting staffing model, HISH CSWs will have consistent hours of work , week over week and have the opportunity to be a respected member of a larger healthcare team striving to provide high quality patient care and positive transition experience for both the patient and their caregivers.

The hours of work will be 7, 12-hour shifts within a 2-week rotation inclusive of weekends :

Two-week Rotation

Mon / Tues / Sat / Sun

0800 - 2000 hrs

Wed / Thurs / Fri

0800 - 2000 hrs

This is an equivalent of seventy-seven (77) hours per bi-weekly pay period!

What The Role Involves

  • Assisting with daily living activities, such as personal care (rise and retire), assistance with household tasks, respite care, medication assistance, special functions
  • Promoting a safe environment by demonstrating proper body mechanics, safe transfer techniques, and home safety
  • Providing cognitive, social and / or emotional support to the patient
  • Preparing nutritious, varied and economical meals and snacks, including special diets, as determined by the care plan, using proper food preparation, storage, and hygiene practices.

  • Ability to work both independently and as part of a team
  • Availability to work non-traditional hours, such as early morning, evenings, and alternate weekends
  • A valid driver’s license and vehicle
  • Valid certificate as a Personal Support Worker, Internationally Trained Nurse, Health Care Aid, Developmental Support Worker, Home Support Worker, or enrollment in a Nursing program
  • To protect our patients, we require current vulnerable sector check screening, and a clear background check
