At Traverse Independence, we offer a wide range of supports and services for individuals with acquired brain injuries (ABI) in the City of Guelph. One of our most exceptional services is the ABI in the Streets program, and we are looking for passionate outreach workers to join our team.
Your Role : As an outreach worker, you will be at the heart of our mission, building trusted relationships with clients and focusing on the social determinants of health. You will assist clients in connecting with mental health and addiction programs, the shelter system, primary care, the HART hub, with some of them in safe, secure housing and others seeking housing. This inspiring program makes a profound difference in the lives of our clients. Together, you will experience the joy and satisfaction of seeing them achieve their goals, improve their quality of life, and cope with their day-to-day activities.
Your Team : You will report to an Outreach Coordinator and work alongside a dedicated team that includes an ABI specialized psychiatrist, occupational and behavior therapist, and an intensive case coordinator. This position focuses on the population in Guelph that accesses the shelter system, supportive, and transitional housing. You will be part of a larger network, collaborating with partner organizations in the mental health and addictions sectors, truly making a difference.
Your Impact : Experience with people with acquired brain injuries is beneficial but not necessary, as we provide comprehensive training both internally and through the Ontario Brain Injury Association. If you have experience working with individuals who are homeless, have mental health issues, or substance use disorders, this is a fantastic opportunity to add an ABI specialization to your skillset.
