Applications are invited for the full-time position of Nurse Practitioner (Registered Nurse in the Extended Class) Reporting to the Director of Care , the Nurse Practitioner provides direct comprehensive care to LTC facility residents with specialized and complex needs based on enhanced clinical knowledge and skills. The Nurse Practitioner practice is an interdependent practice in which the Nurse Practitioner works collaboratively within an interdisciplinary team, however, the scope of the NP’s practice will be beyond that of regular nursing care. RESPONSIBILITIES Provides comprehensive primary care to residents as part of the interdisciplinary health care team. Performs a person-centered health assessment of residents on admission, annually and as needed (including quarterly medication reviews). Completes holistic health histories, mental status and physical examinations. Collaborates and engages with the resident to develop a person-centered plan of care Diagnoses acute and chronic health conditions, collaborating with the physician as necessary. Orders and / or performs appropriate screening and diagnostic investigations, interpreting results and assumes responsibility for follow-up. Engages in and supports a therapeutic nurse-resident relationship that facilitates health and healing. Focus is on resident’s overall quality of life, maximizing independence and comfort, and promoting a positive outlook of aging to the individual and community. Collaborates with resident or SDM, family, and interdisciplinary team in formulating care plans for managing acute and chronic health issues and to provide comprehensive monitoring. Mobilizes resident and family strengths and resources for health and healing. Prescribes pharmacological agents and therapeutic measures, collaborating with the physician as necessary. Consults with and refers to other health professionals and community services as needed. Evaluates the effectiveness of interventions and adjusts plan of care as needed in collaboration with resident or SDM, family, and / or interdisciplinary team. Accurately documents assessment plan, interventions and evaluations of resident care. Liaises with discharge planning services for hospitalized residents to ensure a smooth transition back to the LTCH. Advocates for and provides palliative and end-of-life care. Participates in internal (e.g. Professional Advisory Committees, Quality Committee) and external committees. Promotes knowledge development of clinical staff by integrating best practices in resident care. Provides leadership and involves the interdisciplinary team in quality improvement initiatives. Participate in research initiatives, identify and implement research based initiatives to improve resident care. QUQUALIFICATIONS Provides proof of current registration with the College of Nurses as Registered Nurse in the Extended Class Has experience in Gerontology / Certification from the Canadian Nurses Association (preferred) Is knowledgeable about primary health care issues in long-term care facilities.
