SUMMARY
The Nurse Practitioner (NP) is a primary care provider who operates within their legislative scope of practice as defined by the applicable regulatory body. This role emphasizes continuity of care, coordination across the care continuum, development and implementation of quality improvement plans, and adoption of evidence-based practices. The position involves a 70% focus on direct care and 30% focus on education, leadership, quality improvement, and research.

- The NP reports directly to the Director of Nursing, with clinical oversight provided by the Medical Director.
ESSENTIAL DUTIES & RESPONSIBILITIES
Maintains confidentiality and operates in an ethical manner in accordance with organizational policies and professional standards.Functions as a member of the primary care team, consulting as necessary with other professionals.Engages in ongoing professional development and stays updated on legislation impacting practice.Develops expertise in elder care, dedicating 70% of time to direct resident care.Direct Care Responsibilities :
Provides comprehensive primary care to residents as part of an interdisciplinary team, including unregulated healthcare staff.Enhances continuity of care through collaboration and referrals as needed.Performs detailed health assessments on admission, annually, and as required.Engages residents in discussions about their care plans, participating in regular care conferences.Orders and interprets diagnostic investigations, assuming follow-up responsibilities.Manages acute and chronic conditions through evidence-based interventions.Advocates for and provides palliative and end-of-life care.Maintains accurate and timely documentation of clinical activities.Research, Education, and Leadership (30%) :
Contributes to a safe and high-quality care environment through leadership in quality improvement and innovation.Serves on internal and external committees relevant to professional practice and care standards.Provides formal and informal education and coaching for interdisciplinary team members.Leads quality improvement initiatives and integrates health promotion and prevention strategies into care practices.Promotes evidence-based practice, collaborating on research and translating findings into practical applications.QUALIFICATIONS AND PROFESSIONAL REQUIREMENTS
Current registration as a Nurse Practitioner (Primary Health Care or Adult) with the relevant regulatory authority.Post-graduate training or certification in gerontology and palliative care is an asset.Holds professional liability protection and current CPR certification.Compliance with applicable public health requirements.
