Reports to :   Manager

Hours :   37.5 hours per week; MonFri

Salary : To commensurate with experience and consistent with UHN compensation policy

Status :   Permanent FullTime

Position Summary

As an integral member of the Nursing Team the responsibilities of the CLINICAL COORDINATOR include : utilizing and applying current specialized knowledge and expertise on recent advances in nursing practice to facilitate the effective management of clinical activities and the coordination of in / outpatient care and continuity in patient care management; performing patient care responsibilities; performing teaching / education responsibility; participating actively in Quality Improvement initiatives; participating in academic and research activities / projects; performing administrative responsibilities; performing crossfunctional and / or other duties consistent with the job classification as assigned or requested.

Utilizes and applies current specialized knowledge and expertise on recent advances in Nephrology nursing practice to facilitate the effective management of clinical activities and the coordination of outpatient care and continuity in patient care management.

Qualifications :

  • Completion of a recognized BScN (or equivalent) required
  • Masters degree preferred
  • Certificate of competency (licensure) from the College of Nurses of Ontario
  • Completion of Basic Cardiac Life Support (B.C.L.S.) program Level II Rescuer certification
  • Up to three (3) years of relevant experience including experience working in an advanced clinical practice role
  • Minimum Five (5) years current experience in hemodialysis nursing
  • C Nephrology (C) or the Nephrology Nursing certificate
  • Effective organizational interpersonal and communication skills
  • Excellent decisionmaking and problem solving skills
  • Excellent time management skills
  • Excellent teaching skills with the ability to teach oneonone and / or groups
  • Ability to function collaboratively within a multidisciplinary team and / or independently Ability to work in a selfdirected manner
  • Demonstrated awareness of and adherence to professional and ethical standards
  • Client service oriented with the ability to effectively work with diversity and appreciate that people with different opinions backgrounds and characteristics bring richness to the challenge or situation at hand
  • Computer literacy
  • Research experience preferred
