Ourclient serves more than 96,000 residents in over 1,600 seniors and other communities(long-term care homes, assisted living facilities, independent living, and group homes).Our client is a national organization with a large network of pharmacy fulfillment centersthat allows them to deliver medications in a timely and cost-effective manner. Our clienttakes an active role in working with their home operator partners to promote residenthealth, staff education, and medication system quality and efficiency.Position Summary : The Clinical Consultant Pharmacist (CCP) is responsible to draw from geriatric-focused clinical practice guidelines and collaborate inside multidisciplinary teams todeliver clinical recommendations and process-guidance for a portfolio of residentialcare, assisted living, and independent living homes to optimize for resident outcomes,safety, and regulatory compliance. This is a full-time position with flexibility andopportunity to work-from-home and on-site from the care homes. Training is provided.The CCP works in partnership with the clinical team, operations, home partners, andother departments in the organization. The CCP is responsible for completing clinicalpharmacy activities in alignment with the Residential Care Regulations, CommunityCare & Assisted Living Act, site policies, and our client’s internal policies.Salary range : $52.50 to $57.50 per hour (Paid as a semi-monthly salary with a 5-day work week)Actual pay rate will be based on the individual candidate’s credentials and experiencefor the role. Your Day to Day : Provide clinical pharmacy services to patients in alignment with the Residential Care Regulations, Community Care & Assisted Living Act, site policies, and our client’s internal policies.Carryout medication reviews every six months, complete med cart and med room audits quarterly, lead and participate in Medication Safety and Advisory Committee (MSAC) meetings twice yearly to client sites.Offer and provide education and delivery education for nurses and the elderly when required. Participate in influenza and COVID preparedness activities obtaining dose-appropriate Tamiflu orders and coordinating and carrying out on-site clinics in partnership with nurses at residential care and retirement homes.Develop collaborative practice networks with physicians, nursing staff, health care aides, case managers and other healthcare providers.Follow and adhere to internal policy and processes established by the Clinical Department and the broader organization.Maintain resident and employee confidentiality in accordance to organizational policy and professional / legal obligations. Support best practice medication management processes. Establish and maintain effective working relationships with physicians, nurses, and other care practitioners such as dieticians, social workers with a focus on optimizing medication therapy and wellness. Serve as a representative and leader of the medication management aspect of our client’s services at the homes.
