Position Summary

Nurse Practitioner, Nurse Seniors Health Centre (SHC)

This Nurse Practitioner role in the Seniors Health Centre is primarily responsible for providing clinical care to support to the Long-Term Care (LTC) patients at SHC.

The NP works in collaboration and consultation with physicians, navigators, coordinators, and other members of the inter-professional health care team to prevent avoidable ED visits and hospitalizations, and to support transitions to and from hospital, when needed.

The NP will act as a link between the hospital and SHC, working with the Family Physicians assigned to our residents, to ensure the most appropriate and efficient clinical care provision.

The NP will also support NLOT program development and direction that is agile and iterative, reflective of the dynamic nature of resident care need as well as evolving Ontario Health and Ministry of Long-Term Care priorities.

In collaboration with the Administrator, Director of Care and the Associate Director of Care, you will be responsible for supporting and being part of a high-performance team, ensuring the delivery of quality and safe care for patients and families.

Educate health care team members in best practice standards as it relates to the management of complex and frail geriatric patients.

This role is also key to NYGH’s Ontario Health Team mandate to create a more Compassionate North York, improving population health for our community through more coordinated, integrated care across the continuum.

On a practical level, you will

  • Practice in accordance with Regulated Health Professions Act, Gerontological Nurses Association (GNA) Standards, The Fixing Long-Term Care Homes Act, the Practice Standards set by College of Nurses of Ontario and standards set out for the Nurse Practitioner.
  • Assist in avoiding hospital admissions for current LTC residents who have new, or increasingly complex medical or specialized equipment needs which cannot currently be accommodated in their LTC home.
  • Support placement of eligible high-needs designated Alternate Level of Care (ALC) patients from hospitals into LTC.
  • Enable the admission of community members with specialized needs to LTC homes.
  • Assist in reducing non-urgent transfers to the Emergency Departments.
  • Provide capacity building for LTC staff to develop and enhance competency for acute treatment protocols.
  • Provide assessment, intervention, monitoring and follow-up for acute and chronic illnesses.
  • Provide leadership and promote excellence in patient-centred nursing care through expert practice, education, consultation and research.
  • Actively participate in a Regional NLOT Community of Practice.
  • Provide input and support in the development and implementation of integrated hospital- community-LTC home clinical pathways.
  • Develop linkages with external partners as appropriate to ensure continuity of care.
  • Address medication reconciliation issues upon discharge from hospital.
  • Participates in Quality initiatives and ensure contribution in meeting Quality Improvement Plan (QIP) annual targets.

Qualifications

  • Master’s degree in nursing / Nurse Practitioner (MN-NP) or Masters of Science in Nursing with successful completion of a post-Masters Acute Care Nurse Practitioner Certificate Program from a recognized school of graduate studies required.
  • Current College of Nurses of Ontario registration in the Extended Class (EC) as an NP (adult) or NP (primary health care) required.
  • Current Basic Cardiac Life Support (BCLS) certification required.
  • 1-3 years of demonstrated experience as a Nurse Practitioner preferred.
  • Previous experience in LTC sector preferred.
  • LTC legislation knowledge is an asset
  • Additional specialty education in Gerontology preferred.
  • 3-5 years of relevant acute medicine experience preferred.
  • Demonstrated strong communication and interpersonal skills, with the ability to work collaboratively with physicians, inter-professional team members, and other stakeholders.
  • Demonstrate exemplary leadership skills with exceptional communication skills (written and verbal).
  • Strong leadership, interpersonal and conceptual skills
  • Excellent interpersonal, customer services, analytical, critical thinking, leadership and conflict resolution skills
  • Motivated self-starter with the ability to work independently and collaboratively as part of a team.
  • Stakeholder engagement, collaborative .
  • Demonstrated time management skills with the ability and flexibility to prioritize multiple tasks in a fast paced, results oriented environment.
  • Advanced nursing assessment skills and sound knowledge of clinical therapeutics.
  • Effective patient / family counseling skills.
  • Ability to work with all health disciplines to achieve best practices
