ConnectWell Community Health is seeking a dynamic Nurse Practitioner (0.9 FTE) to join the interdisciplinary team providing primary health care to rostered clients to the NP at ConnectWell Community Health - Lanark site (31.5 hrs / week). This ongoing position provides comprehensive benefit packages and HOOPP Pension.
Position Summary : ConnectWell Community Health is a fully accredited organization. ConnectWell is a community governed, not-for-profit organization, providing primary health care, health promotion and illness prevention services, and developmental and social services. ConnectWell Community Health services are provided by salaried interdisciplinary team members working to their fullest possible scope, in a collaborative practice to address the health and wellbeing needs of the people.
The Primary Care Nurse Practitioner (NP) functions as a member of the Primary Care Team in the provision of primary health care services offered at ConnectWell Community Health. Primary health care services are provided at ConnectWell and in the community, with an emphasis on health promotion, client education and self-management. The RN (EC) must be aware of, understand, and adhere to the Standards of Practice set out by the College of Nurses of Ontario.
Qualifications :
RN (EC) license / NP Primary Health Care Certificate
Current and unrestricted registration as an RN (EC) with the College of Nurses of Ontario
Three years experience in the nurse practitioner role in a primary care setting or community preferred
Demonstrated ability to work effectively in an inter-disciplinary team
Strong clinical skills
Proven commitment to and knowledge of community-based health care, preferably in a rural community and with disadvantaged population groups
Eperience in the development and delivery of health education / promotion programs.
Ability to work evening hours, as required
Ability to provide on-call telephone services on a rotation basis
Valid driver's license and use of a vehicle
Attention to detail, ability to prioritize, and time management skills
Excellent interpersonal, oral and written communication
Reporting Relationship : The successful candidate will report to Primary Care Manager.
